    Please note that acupuncture, dry needling, massage, and shockwave are NOT treatments covered by insurance and are, thus, cash offerings.

    Payments for deductibles, co-pays, and co-insurance are due at the time of service during your visit.

    If you have no insurance coverage or if you are out-of-network, you can find our cash prices listed below.

    Out of network?

    If you have insurance other than those listed above, most insurance providers have some out-of-network benefits. If you have out-of-network benefits, we can provide you with a superbill after your treatment to submit to your insurance.

  • The quickest way to book an appointment is by clicking the “Book Appointment” button on the top right corner of our website. This gives you access to the live availability of all of our providers.

  • Most insurance companies do NOT require a referral for chiropractic care or acupuncture. However, please call your insurance to verify your coverage prior to scheduling your appointment with us.

  • Unfortunately, we do not. We apologize for any inconvenience this may cause.

    However, we can refer you to another trusted provider who does accept Medicare.

  • Our main entrance is at 400 Commonwealth Ave.

    You will be able to buzz in by dialing #-1-2 on the keypad to the right of the door. If you are not buzzed in immediately, please wait 30 seconds and try again.

    Once you’ve entered the building, take the elevator or stairs on the left in the lobby down to the Ground Floor.

    Metered parking is available on Commonwealth Ave, as well as on the Newbury Street extension by the Harvard Club.

    The closest public transit location is the Hynes Convention Center on the Green Line.

Service Rates

We accept insurance for chiropractic appointments. Acupuncture, dry needling, massage, and shockwave are cash services.

You can also use your HSA or FSA to pay for all treatments received here at Forward (including all cash services!)

  • These are our cash rates for visits. If you use insurance or out-of-network benefits, your amount due will vary depending on your insurance coverage and their allowable reimbursement amounts.

    In-office/Telehealth:
    New patient evaluation and treatment (60min) - $200–$215
    Follow-up (45min) - $175–$185
    Follow-up (30min) - $130–$140

  • These are our cash rates for visits. If you use insurance or out-of-network benefits, your amount due will vary depending on your insurance coverage and their allowable reimbursement amounts.

    Telehealth:
    New patient evaluation and treatment (60min) - $175
    Follow-up (45min) - $115
    Follow-up (30min) - $80

  • New patient evaluation and treatment (60min) - $200
    Follow-up (45-60min) - $150
    Add-on to chiropractic treatment - $50

    Acupuncture added to a chiropractic visit may extend your visit between 15-30 minutes but can usually be done within the allotted visit time.
